Pre-Booking COVID19 Health & Safety Questionnaire

  1. Has the student had symptoms of cough, fever, high temperature, sore throat, runny nose, breathlessness or flu like symptoms now or in the past 14 days?
  2. Has the student been diagnosed with confirmed or suspected COVID-19 infection in the last 14 days?
  3. Has the student been in close contact with a person who is a confirmed or suspected case of COVID-19 in the past 14 days (i.e. less than 2 metres for more than 15 minutes altogether in 1 day)?
  4. Have any of your family been advised by a doctor to self-isolate at this time?
  5. Have any of your family been advised by a doctor to cocoon at this time?
  6. Please provide details* to us by email if any other circumstances relating to COVID-19, not included in the above, which may need to be considered to allow your safe return to work.

* If you are unsure whether or not you are in an at-risk category, please contact your GP. If your situation changes after you complete and submit this form, please inform the school immediately.

If the answer to any of the Questions Above is YES please do not book this course.

NPAS Summer Camp Checklist

We really want the children and young people attending our school to have the best time possible, and to enjoy a full NPAS experience. We can now do this by adhering to the Guidelines set out by the HSA, the HSE and the Department of Education. In addition to the various government documents and guidelines we have some specific new rules for each of our school venues.

  1. Please read our Covid safety policy and our Covid Questionnaire before booking. They can be found at ‘SAFE@NPAS’ on our school website https://www.npas.ie/covid-19-health-and-safety/
  2. No parents will be allowed inside the Community Centre. NPAS Staff will collect Students from outside the main door of The Community Centre and return them there at pick-up.
  3. Drop off and Pick up times are very important and must be adhered to.
  4. All students must be pre-registered and signed up for class. There will be no drop-ins or additions to classes on the day due to contract tracing.
  5. There will not be any reception facilities at The Community Centre but NPAS Staff will be there to answer any questions you have.
  6. Please bring your own bottle of water and snack. Please make sure that the student can open their own lunch.
  7. Please wear slip on shoes if a student can not tie their shoe laces.
  8. Please wear a jacket. The Community Centre has a wonderful outdoor area which we may use weather permitting!

Again, the above points will enable us to open safely for all of our Students, their families and our teachers and staff. They are not recommendations, they are mandatory and we are obliged to make sure that all measures are followed and adhered to.
